Should I publish prices at all?

Yes. A gym that hides its price is one people assume is expensive, and they find out from a competitor who did publish.

How many tiers?

Do I have to mention the joining fee?

It is the single most complained about number in this trade, and complaining happens because it was a surprise. On the page it is just a term.
